Congratulations on receiving your job offer from us. Here you will find all the information to begin your new role with us.

In preparation for your start, we will need some new starter forms completed and returned. Please download these files by clicking on the highlighted form name and return them to us via e-mail to Wrecruitment@Watfordmencap.org.uk 

• DBS Disqualification Declaration Form – this is to declare if you, or anyone you live with, are disqualified from working with children, as you have stated that you are interested in working with Watford Mencap with children and adults. This form also authorises Watford Mencap to register you with the DBS update service (which is a contract requirement for children’s services) and to allow Watford Mencap to check your DBS on the update service. 

• Personal Details Form – this provides us with your contact details, your bank details (so we can pay you) and your emergency contact details should we need to contact someone if you become unwell at work etc.

 

• Vehicle Driver’s Questionnaire Form – this is to record details about your driving such as your driver’s licence, a driving and vehicle risk assessment. This is required for any staff who may use their own car to drive from one business location to another (e.g. to attend meetings or training) for which they are eligible to claim mileage. I will need a copy of your car insurance showing business use. 



• Health Declaration Form – this is to identify any existing medical conditions that may require adjustments. 

• HMRC Starter Checklist Form - is an HMRC form which can be filled in where you don’t have a P45. You can complete this and then submit a P45 if you get it at a later date.

• Covid 19 Risk Assessment Form - this risk assesment form so we can adentify if you fall into the high risk categories and enable us to make suitable arrangements if necessary.

• Covid 19 Vaccination Form - As social care staff you are elibile for Covid Vaccinations, this not mandatory but reccomended in order to keep you and our clients safe. Please complete this form to indication if you've received any Covid 19 vaccinations.
